Medication Therapy Management (MTM) is a personalized service provided to you by qualified and specially trained Pharmacists. Care Dynamics Senior care was founded, and is managed, by a board certified geriatric pharmacist who is licensed as a consultant pharmacist. The goal of MTM is to curb and eliminate medication related problems, while providing optimal health and cost benefits to you. At Care Dynamics, our services are designed to help you understand how to take your medications as prescribed. We will take a look at your medication history to make sure there are no drug-drug interactions and/or side effects with your drug regimen.
2) How Does MTM Services Work?
A specially trained Pharmacist will work closely with you and your other health care providers to ensure that your prescriptions, over-the- counter drugs, vitamins and herbal supplements are working well without adverse drug-interactions. They will ensure regular follow ups to make sure you get the results you want from your medications.
3) What are the benefits of MTM?
Many patients take more than four (4) medications from different sources which may include: more than one pharmacy, mail-order, samples from doctor’s office and herbal supplements. You will receive increased understanding of your medication and experience superior care with a specially trained Pharmacist dedicated to overseeing all your medications from every source. Ultimately, you will become more knowledgeable about the medications you are taking and can participate actively in medication therapy decisions.
4) What Does MTM Cost?
All Medicare Prescription Drug Plan enrollees are eligible. Even if you or your loved one's insurance plan doesn't cover MTM services, we are happy to provide services at affordable rates.
5) Who is Eligible for MTM?
Patients currently taking four (4) or more prescription medications and have at least two (2) chronic health conditions. Patients who see multiple prescribers. Patients who take medications that are higher risk. Patients who are transitioning from the hospital back to home. Patients who self-identify need for MTM who wants to reduce out of pocket medication costs. Patients with change or loss of insurance coverage. Get a Referral Form from your Primary Care Provider (we may help you obtain this upon request).

7) What should I expect in my first MTM meeting?
You should have with you all medication bottles or have a detailed and complete list of your current prescriptions written down including: over-the-counter herbals, vitamins and supplements. Our Pharmacists will ask you questions about your medical history, current and past medication use and medication challenges you face. This information will be documented and help us to identify any problems with your medications. Your initial telephone interview may last anywhere from 30 minutes to 45 minutes. With your consent, our Pharmacists will contact your Primary Care Provider and present an assessment based on evidence-based guidelines and recommendations. Our Pharmacists will work closely with your Primary Care Provider to make your healthcare as effective as possible and to resolve any medication-related problems.
It is recommended that you see your MTM Pharmacist three (3) times during the first year (more if you and your pharmacist feel additional visits are needed) to provide ongoing assessment of your medication regimen
8) Do you Bill Insurance Companies?
A standard Insurance Claim Form completed with appropriate codes will be provided. We will collect reimbursement from your health insurance provider.
